The platform is built to support member agencies by ensuring seamless data exchange capabilities, as outlined in the Tailored Plan CMA manual. It integrates effectively with all LME/MCOs, the North Carolina Health Information Exchange (NC HIE), and other value-added data exchange systems—ensuring compliance, interoperability, and enhanced care coordination across the state.

Our secure interoperability bridge enables seamless and HIPAA-compliant data exchange across payer and provider platforms, ensuring that essential clinical and administrative information flows efficiently across the care continuum. Our solution has the capacity to integrate data from a variety of sources (HIE, EHRs, Wearables, etc) and file types.

By linking utilization patterns with risk indicators, our population health tool supports proactive care management strategies that reduce costs and improve outcomes. It enables providers to take shared accountability for the health of their communities.

Providers across the network can access standardized dashboards that track key quality metrics and benchmark performance against Collaborative Health Network peers, state-level data, and national standards—driving continuous quality improvement.

Our data analytics platform enables the tracking of clinical outcomes, utilization patterns, and financial metrics across the network. These tools support performance improvement, ensure compliance with value-based care models, and provide actionable insights for operational decision-making and stakeholder reporting.

Our network management platform provides real-time visibility into provider service capacity as well as integrates with existing bed management solutions. The referral management solution supports both intra-network and external referrals—ensuring patients are connected to the most appropriate care while optimizing care transitions and clinical decision-making. Referrals can be submitted via existing referral management protocols (ie; 360X, fax, and direct electronic submission)
