We are the largest Tailored Care Management behavioral health network of providers across North Carolina, serving more than 50% of the NC tailored care management population not directly assigned to Tailored Plans.

Our providers bring a diverse range of evidence-based clinical expertise across both behavioral and physical health disciplines.

We leverage the strengths of our Certified Community Behavioral Health Clinics (CCBHC) designation, integrate peer support from individuals with lived experience, and deliver a comprehensive crisis continuum of care to enhance access, quality, and outcomes across our network.

Jasmine Sandoval, DBA

Jasmine Sandoval serves as the Director of Network Operations for Collaborative Health Network and has extensive experience in healthcare operations, project management, and organizational leadership. Her professional experience includes healthcare technology, practice management, human resources, and business operations, with a background working across both startup and established organizations. She has led cross-functional initiatives, supported growth and change, and developed solutions to strengthen operations and organizational performance.
Jasmine holds a Doctor of Business Administration and a master’s degree in Human Resources Management. Her diverse professional background shapes her approach to leadership, with an emphasis on collaboration, adaptability, and developing solutions that meet evolving organizational needs.
Throughout her career, Jasmine has focused on building effective processes, supporting teams through change, and creating environments that support long-term growth.
